The Beserker is an ancient being in fate some of divine ancient beast and ancient monsters have a godly mech kinda esthetic.

The goddess is neither a Servant nor a homunculus. She simply possesses a homunculus's body in order to manifest in this world.
As for her origin, she comes from the same era as Gilgamesh and Enkidu, and she has a grudge against both of them. By the way, this True Berserker also has a grudge against Gil and Enk. They're all from the same period.
This goddess wasn't summoned by the Holy Grail. Instead, the appearance of Gilgamesh and Enkidu allowed her to manifest in the world.
And yes, Servants can be summoned from the past, present, future, and even from different timelines. It's also possible to summon different versions of the same Servant, as long as those versions have been recorded separately in the Throne of Heroes.
For example, imagine summoning LeBron James as both his 23-year-old version and his 45-year-old version. Why? Because he may have left his mark on history twice: once at 23, leading to a first registration in the Throne of Heroes, and again at 45, after accomplishing something even more remarkable, resulting in a second registration.
Of course, it's a bit more complicated than that. Different versions of a Servant are not always just older or younger versions of the same person. Sometimes they represent who that person was during a specific stage of their life: a wise, gentle, and kind individual at 23, then a cold ruler or even a dictator at 45.
As for their names and roles, you'll have to wait until Season 2 or 3 to find that out.
