To make something clear, the Kuruokas were trying to summon Qin Shi Huang, the founder of the Qin dynasty and historically the first emperor of China.
The Godfelling Crossbow was the catalyst they were going to use, but Tsubaki summoning her Servant by accident ruined that. While the exact details of the story differ, Qin Shi Huang used the crossbow to kill a sea god that was causing troubles for him, and that the sea god had the appearance of a shark.
"Jiao" is essentially the remnants of that sea god, now tied to the weapon that killed it.
It's not really made clear in the anime, but Jiao is basically a ghost-type spirit attached to the relic that Tsubaki's parents WOULD have used to summon their Servant, if Tsubaki hadn't gotten to the summoning first.
About the Four Horsemen of the Apocalypse: I can understand your confusion, since Pale Rider is called both Death and Pestilence, but here's the thing;
Fate uses the Original Four Horsemen of the Apocalypse, which are War, Famine, Conquest and Death. Pestilence started to get real popular during the 20th century, so most pop culture stuff replace Conquest with it, which, to be fair, does make a lot of sense. Pestilence definitely deserves to be its own Horseman more than Conquest, so Conquest usually just gets fused into War. But here, diseases are just another way that Pale Rider can spread death around.
Just something to note here, Tsubaki's wording of "To be a magician" is very important here.
Because in the nasuverse, a magician is very different from a magus or magi. Magus or magi is usually the term referred to most people who utilize magecraft, like Waver and Flat. Magicians meanwhile are able to utilise magic, true magic.
To describe magic, it's essentially miracles that is not replicable by humanity no matter the resource or time given. Things like traversing parallel worlds and time manipulation.
So the fact that Tsubaki chose to say magician rather than a magus is what Jester was fishing for. Since Pale Rider can't exactly fulfill that wish itself, it decided to finally join the fray and win the grail for Tsubaki.
It's honestly funny that The troublemaker duo were only able to enter the reality marble because Cerberus became apart of it, and Cerberus can be distracted with sweets allowing one to come and go from the underworld.
As a lot of people said Jiao is a sea god slain by Qin Shi Huang but fun fact this slaying was performed in order for one of QSH’s vessels Xu Fu to travel east out to sea to search for a means of immortality.
And if this sounds familiar it’s because this could be the direct or indirect inspiration behind Hells Paradise a mysterious island housing the secrets of immortality.
If memory serves in the second season I believe Xu Fu is even name dropped as the creator of the Tensen who continue his work researching the means for immortality.
On the other hand fate’s depiction of Xu fu didn’t go east searching for immortality but was instead looking for a means to kill an immortal (a long story in of itself).

The wording Tsubaki used is very important. "I want to help out mommy and daddy by becoming a MAGICIAN!"
In Fate a Mage and a Magician are two different things. Mages practices magecraft, which uses magical energy to reproduce scientifically possible phenomena through conventional means. A Magician operates at a higher level, wielding True Magic to achieve "miracles" or effects impossible by science. The "easiest" way to gain True Magic and thus become a Magician is through the Holy Grail which you need to kill the other participants for.
So Jester basically manipulatated Tsubaki to activate an end of the world to kill everybody in the Reality Marble.
